PROGRAM DESCRIPTION:
Individualized Planning Model which entails coordination of support/supervision with individuals and families within the community; Independent Living with intensive supports (24 hour supervision), living alone or shared apartment, Community Training Home with live-in trained support staff/family. Day Support Options; provides support with socialization/recreational activities in the community or in your home. |
