PROGRAM DESCRIPTION:
Person Centered Planning Model which entails Coordination of Home Sharing with individuals and families within the community; Independent Living with intensive supports (living alone, shared apartment identified through the program); or self-directed management of own household with live in trained support personnel as options being offered to the Individual Participant.
SERVICES PROVIDED:
Person Centered Planning Process which fosters Self Governance, Self Advocacy, Individual Decision Making in Housing and Activities of Daily Living arenas.
Assistance with Applications for Rent Subsidies, Securing Federal Entitlements
Coordination of Nursing, Medical and Psychiatric Counseling Services
Provision and support of Social and Recreational outlets and activities
Public Transportation Training
Education in Health and Safety, Good Diets, Shopping Habits, Budgeting Money
Enhancement of networks and developing quality lifestyles within the community
